Everybody prepared you for the birth. Then the birth happened, you came home, and it turned out that was the beginning rather than the end.

The first twelve weeks after birth have a name — the fourth trimester — and understanding what's happening makes it less frightening, even when it doesn't make it easier.

What your body is doing

Your uterus is shrinking from roughly the size of a watermelon back to a pear. Your blood volume drops substantially. Your hormones fall further and faster in the days after birth than at almost any other point in your life.

That last one is why the third or fourth day so often arrives with tears from nowhere. It isn't a character flaw and it isn't ingratitude. It's an endocrine event.

Common, even though nobody mentioned it

  • Night sweats that soak the sheets for a couple of weeks
  • Hair coming out in handfuls around month three
  • Afterpains while nursing, often stronger with a second or third baby
  • Crying on day three or four for no reason you can name
  • Feeling like a stranger in your own body
  • Loving your baby completely and grieving your old life at the same time

Please call someone about these

  • Soaking a pad in an hour or less, or passing clots bigger than a plum
  • A fever, or a red, hot, painful area on your breast
  • A headache that won't shift, vision changes, or sudden swelling in your face and hands
  • Calf pain, or chest pain and shortness of breath
  • Sadness or anxiety that isn't lifting after two weeks
  • Any thought of harming yourself or your baby

That last one especially. It is far more common than anyone says out loud, and it is treatable. Please tell someone today.

What women tell us actually helps

Someone else holding the baby every day so you can shower properly. Meals you didn't cook. Ten minutes of daylight. Saying yes when people offer, and being specific when they ask what you need.

And lowering the bar, all the way down. The house does not need to be clean. Nobody is coming to inspect.

You don't have to do this alone. You were never meant to.

Questions women ask

How long does the fourth trimester last?

About twelve weeks, though many women say full recovery takes considerably longer — often closer to a year, particularly for the pelvic floor and for hormones while breastfeeding.

Is it normal to cry a lot after having a baby?

It's very common. The 'baby blues' affect most new mothers in the first two weeks and are driven by a genuine hormonal shift. If it hasn't lifted after two weeks, or if it's deepening, that's worth a call to your provider.
